Crisis Resolution Home Treatment team are currently looking to recruit band 3 support workers to the team. CRHT operates on a 24 hour, 7 day a week basis.
CRHT offers to provide intensive home support to services users as an alternative to hospital admission.
CRHT works collaboratively with a variety of services to support service users at home. CRHT is based in Broadoak Unit and will cover Liverpool and Sefton areas.
The post holder will be expected to work on a rotational shift basis in accordance with service demands, this includes working weekends and night shifts.
Main duties of the job
The post holder will work to assist with the continuity of care for service users within the appropriate care setting. The post holder will work from the community environment.
The post holder will provide support, give time to an allocated group of service users, and aim to promote recovery predominantly within the community environment.

Job responsibilities
1. Facilitate and support the development of appropriate personal care.

6. Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff.
7. Participate in regular supervision, engage in reflective practice and be committed to continued development.
8. Provide information to enable the effectiveness of care to be evaluated.
9. Ensure appropriate records are maintained under the direction of the qualified practitioner.
10. Contribute to the involvement of service users and carers/relatives in the delivery of care.
11. Provide reassurance and support to service users and carers/family.
12. Work collaboratively within the care team to ensure that service users' needs are met.
13. Recognise and respond appropriately to urgent and emergency situations.
14. Contribute to the development of services.
15. Contribute to the effective and efficient use of resources.
16. Help to identify early signs of relapse as described within the care plan and alert the appropriate staff involved in the service user's care.
17. Understand and adhere to Trust policies, procedures and clinical guidelines.
18. Report any concerns regarding service user care to the qualified practitioner.
19. Contribute to service improvement initiatives to enhance the quality of service user care.
20. Participate in service user and public involvement activities.
21. Support individuals' equality, diversity and rights.

Person Specification
Knowledge/Experience
* Ability to function effectively as a team member.
* Experience of working with persons within the appropriate care setting and their carers.
* An understanding of the organisation of nursing care.
* Basic understanding of NHS issues.
* Basic awareness of the Mental Health Act.
* Safeguarding children/vulnerable adults guidelines.
* Overview and knowledge of acute mental health related issues.
Qualifications
* NVQ 3 or equivalent level of experience within a care setting.
Skills
* Ability to communicate and engage with service users.
* Able to present factual information and refer questions to others where appropriate.
