Would you like to work as part of the hospice in home MDT and Hospice MDT? We have various hours available which will be discussed at interview.
You will take on the role as shift leader ensuring the shift runs smoothly.
You will be responsible for triaging calls over 24 hours and giving advice and support to patients who are known to St Helena or have their details recorded on the My Care Choices Register.
You will also provide rapid response visits to patients in crisis in the community.
The role will involve being a clinical resource to Staff nurses, and Health Care Assistants in the team, Administrators and Call Handlers within SinglePoint.
You will work closely with community and primary care colleagues, St Helena and hospital staff in order to ensure that patients requiring end of life care receive an integrated service in a timely manner.
The role will include working overnight, taking calls from patients and professionals and undertaking home visits as necessary throughout North East Essex.
Main duties of the job
1. As a registered nurse/paramedic, providing high standards of holistic, individualised care and support to patients and families, through a process of assessment, planning, implementation and evaluation of care needs.
2. To act as a resource to patients and their families in the community, providing telephone advice and support.
3. To provide a rapid response visit to patients with urgent palliative or end of life care needs within the last 3 months of life.
4. To be responsive to changing patient needs and flexibly visit according to suggested triaged response time.
5. To work shifts including overnight, triaging calls and visiting patients as necessary, often as a lone worker.
6. Take responsibility for accurate assessments of patients and families and to provide ongoing plans of care for St Helena staff and other healthcare providers.
7. To work with the Hospice in the Home Matron / Clinical Nurse Manager to plan and implement change and development.
8. To provide supervision and work alongside Healthcare Assistants and provide guidance and expert advice, assisting in the identification and meeting of their educational needs.
9. To ensure end of life procedures are carried out sensitively and appropriately.
10. To verify death following training and when it is appropriate to do so in line with St Helena policy.

10. To maintain and promote effective and efficient working and communication with other community professionals, hospital professionals, voluntary, private and statutory agencies and other hospices.
11. Participate in the audit process and encourage colleagues to embrace the opportunities and improvement gained from the audit cycle.
12. To ensure end of life procedures are carried out sensitively and appropriately.
13. To verify death following training and when it is appropriate to do so in line with St Helena policy.
14. Demonstrate awareness of emotional needs of patients and families and the sensitivity and communication skills to meet these needs.
15. Demonstrate effective communication with patients key workers in order to ensure continuity of care for patients across all sectors.
16. To maintain professional awareness of current developments in nursing and palliative care and evidence based practice in order to remain an effective practitioner.
17. Follow St Helena policy at all times.
18. Participate in Clinical Supervision both as a recipient and a provider.
19. Comply with NMC guidelines.
20. Be aware of own emotional needs, relating to the palliative care environment, and how to meet them.
21. To act as an advocate safeguarding the patients and families rights and interests, abilities and aptitudes, encouraging empowerment and informed choice.
22. To communicate sensitively and effectively both verbally and in writing with patients, families and colleagues including volunteers. In some cases there may be barriers to understanding or highly sensitive communication required.
23. Ensure you follow all policies and procedures in relation to prevention and infection control.

* Registered Nurse
* Registered paramedic with the HCPC
* Evidence of further palliative care education
* Degree in relevant subject

* Teaching qualification

Experience
* Significant post registration experience at Band 5 equivalent or above in any setting, working with palliative care patients
* Experience of working in the community
* Experience of working with people with mental health needs
* Experience of working with people with sensory and learning disabilities
