Pay: Dependent on Experience (ARRS Funded) Main duties of the job Supporting the practice by being one of the duty clinicians alongside other ANPs and GPs. General health and wellbeing promotion as well as creating individual, person-centred plans for patients with, or at risk of, long term condition. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effecting emergency care. Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met. Meeting the needs of patients presenting for opportunistic wound care. Practicing autonomously as an Advanced Nurse Practitioner providing patient centred clinic care. Process and communication results of tests to patients and ensure that a care plan is in place. Maintain accurate, up to date patients records on the practice computer system (SystmOne).
