JOB TITLE: Senior Support Worker / Team Leader
SERVICE: Portland Supported Living
RESPONSIBLE TO: Community Service Managers / Assistant Managers / Heads of Service
RESPONSIBLE FOR: Support Workers, safety of people we support, delivery of person-centred care and direct line management of a small pool of support workers.
KEY CONTACTS: Support Staff, Families, Social Services, Health Professionals, Voluntary agencies.
RATE OF PAY: £12.80
Purpose of Role:
To ensure the smooth running of the day-to-day activities within each environment (service or house), to lead, coach and mentor support staff, ensure medication is safe and managed on a daily basis, and that activity planners and menu planners reflect support plan goals and aspirations.
Main Duties and Responsibilities:
1. To coach and lead the staff team, completing supervisions with Support Workers, huddles, and mentoring when on duty with other junior colleagues.
2. To complete audit checks on records and ensure that any areas highlighted are addressed through team meetings and supervisions.
3. To effectively manage the service rotas, ensuring adequate cover is provided, liaising with agencies where required and working under the direction of the community services manager.
4. Ensure that each individual has a person-centred support plan that is reviewed in accordance with our vision and values.
5. Ensure that risk assessments are completed on all identified risks for each individual and the home and these are reviewed in accordance with the organisation's policies and procedures.
6. Ensure that any accidents or incidents are reviewed with clear outcomes in accordance with the organisation’s policies and procedures.
7. Complete checks on all financial transactions and ensure that all monies of people supported are safeguarded.
8. To oversee ordering, booking in, and day-to-day management and audit oversight of medication for people we support, those with capacity or without.
9. To complete monthly home audits/checks in accordance with the organisation’s policies and procedures.
10. To work in partnership with a wide variety of agencies to ensure that all needs are met.
11. To undertake direct support within the services as agreed on the rota.
12. To undertake any other duties as required by the Registered or Regional Director.
13. To support with probation reviews, investigations, and formal disciplinary meetings and to act with a high standard of confidentiality during these times.
14. To oversee activity planners, healthy eating plans, documentation, and end-of-month oversight as a service.
