Job overview
An opportunity has arisen for an experienced and dynamic Mental Health Nurse to join the Positive Lifestyle Team on a full time basis.
The successful applicant will work across the Greater Preston and Chorley/South Ribble Clinical Commissioning Group footprint. The role will involve working with Service Users who have a Severe Mental Illness and referred both from the CMHT and GP’s but require more interventions around Physical Health Monitoring and improving their quality of life as well as monitoring mental health. It is anticipated that the successful applicant will have a caseload of Service Users from both Primary and Secondary services.
Main duties of the job
The successful applicant will be able to work well within a team as well as independently and have excellent liaison and communication skills. Working across both the Primary Care Team and Mental Health Services means they will need to be flexible and adaptable and able to form good working relationships with others.
Regular supervision and CPD opportunities are available.
