The East Community Mental Health Team are looking for a Mental Health Practitioner to join our established team as an Assessment and duty worker.
We would welcome applications from RMN, OT and Social Workers who have experience of working with people suffering severe mental illness, whether this be inpatient or community based.
You would work alongside other duty workers who work together to triage all referrals that come into the CMHT (with the support of a duty manager and psychiatrists), gather more information and arrange assessments. The role also entails telephone calls from people who are open to the service and the CCO/Medic is unavailable.
The team is a very supportive team, and you will have case management supervision, clinical supervision, and opportunities for training.
Main duties of the job
1. To manage and triage referrals into the service, planning and providing timely assessments.
2. The post holder will clinically manage the referrals and assessments in line with the clinical pathways within the CMHT.
3. The post holder will review cases identified by the Team Leader, involving reviewing appropriate interventions following review.

Salary: £37,338 to £44,962 a year based on full-time hours.
