What You Will Do: Patient Assessment and Support : Conduct comprehensive assessments for patients at the front door of the Emergency Department, ensuring the effective planning of patient pathways and discharge processes. Equipment Assessments : Evaluate and ensure that the necessary equipment provisions are in place for patient care, promoting their comfort and safety. What We Are Looking For: HCPC Registration : You must be a registered Occupational Therapist or Physiotherapist with the Health and Care Professions Council (HCPC). Experience : At least two years' experience in front door admission avoidance, rapid response areas, or discharge planning. Driving Licence (Advantageous) : While not essential, having a valid driving licence and the ability to travel between sites would be beneficial.
