We are recruiting for a full time Patient Flow and Quality Coordinator to provide efficient administrative systems to support the referrals, admission and discharge process, as well as key local and contract compliance reports across the hospital. You’re day-to-day… Responsibility for dealing with all PICU service line referrals, from the initial referrals to ensuring that a decision is made within the hour timeframe. This involves engaging the MDT to ensure the appropriate information is received and the decision is made. Responsibility for dealing with referrals for the three other service lines within the hospital, from receiving the referral, to liaising with the MDT and nurse assessor to arrange assessment of potential patients. Supporting all wards to arrange the admissions of new patients, liaising with other hospitals and external professionals if required. Supporting wards to arrange discharges, liaising with other hospitals and external professionals if required. Booking & securing funding for transport for patient within this hospital. Administrative support for data and information systems regarding the referrals, admission and discharge process and ensuring these systems are kept up to date with accurate patient information. Creating a weekly report on referrals, admission and discharges and presenting this in the Heads of Department meeting. Working with the Business Relationship Manager to support with arranging meetings and providing information. Supporting the Quality Assurance and Contracts lead with completing monthly and quarterly local and contract compliance reports, liaising with other teams throughout the hospital (HR, finance, clinical staff) to ensure correct information is inputted and submitted within the deadline. Completing patient satisfaction surveys, both during a patient’s stay and when discharged. Daily attendance at the safety huddle meeting, to report on referrals, admissions and discharges within the past 24 hours. Other administrative support for the Quality team, including completing regular audits locally to the hospital, minuting meetings and collating action plans to be shared within the OLAP. Why Cygnet? Ability to set up efficient administrative systems for large numbers of work streams. Experience working with electronic patient clinical records and databases. Ability to work on own initiative and to deadlines. An ability to minute complex meetings. Knowledge of CQC, KLOEs, and CQC compliance.
