Job summary
Pharmacy technicians play animportant role, complementing clinical pharmacists, community pharmacists andother members of the PCN multi-disciplinary team. The pharmacy technician willprovide support to the network as a whole and each member practice asneeded. The aim is to improve thequality of prescribing and patient outcomes across the network through highquality medicines review and optimisation.
Main duties of the job
Pharmacy technicians core roleresponsibilities will cover clinical, and technical and administrativecategories; supporting improvements to maximize safe, cost effectiveprescribing and achieve the best clinical outcomes for patients. The postholder will be involved in many aspects of medicines management and besupported and developed in this role by the PCN clinical and pharmacyteams.

1. Workclosely with the practice teams, pharmacy colleagues and patients to ensurecoordinated and efficient processing of prescription requests synchronising medication, setting up electronic repeat dispensing, monitoringmedicines use to prevent waste or over usage
2. Carryout medicine reconciliations for discharge summaries, clinic letters and othertransfer of care communications
3. Workwith clinicians and the pharmacy team to develop and implement robust systemsfor shared care prescribing and high-risk drug monitoring and other drugmonitoring for optimisation of long-term conditions
4. Updateand maintain accurate and comprehensive patient medication records
Improving Patient Safety, Audits, and SupportingReviews
5. Work with the pharmacy team and clinicians todeliver QIPP/PIS and other local commissioned enhanced services to improvemedicines optimisation
6. Work alongside the pharmacy team to monitor andaction safety and drug alerts in a timely and appropriate manner, includingliaising with patients when necessary
7. Work with clinicians and pharmacy team toundertake medicine audits and other quality improvement/ project work asrequired
8. Work alongside the admin team to promote anddevelop high quality prescribing processes including identifying qualityimprovement opportunities

9. Develop positive and professional workingrelationships with colleagues to ensure excellent patient care
10. Promote best practice for medicines useincluding supporting training of practice teams on medicines processes andpolicies
11. To respond to medication queries or requests formedicines information from patients and staff in a professional and timelymanner including escalating to clinicians or senior pharmacy colleagues whennecessary
12. Work in practice with practice teams support the achievementof prescribing outcomes for Quality Outcomes Framework

Qualifications
Essential
14. Qualified Pharmacy Technician
15. Current registration with General Pharmaceutical Council (GPhC)
Desirable
16. Enrolled in or completed approved training pathway.
17. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH).
18. Completed CPPE Consultation Skills Module or equivalent.
Experience
Essential
19. Evidence of continued professional development (CPD).
20. Experience of working as a qualified, registered technician in primary care, community or hospital pharmacy.
21. Understands the aims of current healthcare policy and appreciate the implications of this for clinical pharmacy in PCNs.
22. Knowledge of the principles of medicines optimisation.
23. Knowledge and understanding of pharmacy law and ethics and current legislation.
24. Good clinical pharmacy knowledge including terminology.
25. Experience of providing pharmaceutical and prescribing advice.
26. Experience of clinical audits.
27. Experience of delivering education and training.
28. Ability to analyse and interpret prescribing data.
Desirable
29. Experience working as a qualified, registered technician in primary care ICB, GP practices, PCN.
30. Experience using EMIS clinical software.
