Job Responsibilities
The Urgent Community Response Service in Woodbury, Exmouth, and Budleigh Salterton is looking to recruit an Urgent Community Response Support Worker. This service provides support to people in their own homes in times of crisis, to avoid unnecessary hospital admissions and facilitate discharges home from hospital.
The service operates 7 days a week, with shift patterns covering the hours from 07:30 to 22:00. You will be part of a team providing a quality service promoting independence, dignity, and support.
Previous community experience is desired but not essential; the team will provide the support and guidance you need to excel. You will join a dynamic multidisciplinary team, working with Health and Social Care Teams to respond to urgent situations and ensure timely discharge from Acute and Community Hospitals.
Successful candidates will have a Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent qualification or significant equivalent experience working with older people in a community setting. Competency with IT is essential, as electronic recording is part of the role. A driving licence and access to a vehicle are also essential.
Urgent Community Response Support Workers are lone workers within the community, although support is always available.
Working Pattern: You will need to work a range of shifts between 07:30 and 22:00, including weekends and evenings at enhanced rates.
