Main duties To provide care to patients at the point of entry to primary care, including those with undifferentiated and undiagnosed health problems. To use advanced nursing skills to take a comprehensive health history, examine, diagnose and provide a management plan within agreed national and practice protocols. To manage commonly occurring minor illnesses. Prescribe as an independent nurse and supplementary prescriber. Recognise abnormalities and screen patients for disease and early signs of illness. Initiate laboratory investigations, interpret results and take appropriate action. To initiate, record and interpret ECGs and take appropriate action. To make referrals to other healthcare professionals after discussion with the GP. Provide an adult and child immunisation service working to practice protocols and patient group directives. Take swabs for microbiology. Practice good infection control procedures and apply measures in accordance with local and national policies; advise colleagues and patients on infection control measures. undertake safeguarding training and ensure up to date knowledge on recognising signs and symptoms of abuse, good record keeping and liaison with other agencies, local procedures and point of referral. Recognise psychological needs of patients presenting with mental health issues, depression, suicide ideas; be aware of appropriate mechanism for referral for family violence, vulnerable adults, substance misuse and addictive behaviour. Keep accurate and timely records pertaining to all clinical encounters with patients in accordance with practice protocols and template use and in line with NMC guidelines. Participate in audit and research as required to fulfil contractual requirements of the practice GMS contract and to facilitate the monitoring and improvement of nursing services. Implement evidence-based practice. Maintain an up-to-date clinical knowledge and participate in CPD; keep an up-to-date learning plan which can be reviewed and agreed at annual appraisal. Maintain competence in resuscitation and anaphylaxis technique. All other duties that can reasonably be expected to ensure the smooth running of the practice and continuity of service to patients.
