Advanced Specialist Pharmacist Elderly Care

Are you looking for a new challenge and are passionate about the area of elderly care? This is an exciting opportunity to join the elderly care team and support this complex polypharmacy and rapidly growing population group.
You will lead on all aspects of medicines optimisation and deprescribing within the elderly care speciality, supporting medical and nursing teams, as well as allied health professionals in the safe and effective use of medicines.
Main duties of the job
1. Providing a clinical pharmacy service to the elderly care wards including ward round attendance to support medicines optimisation and deprescribing initiatives for the polypharmacy population.
2. Training and development of rotational junior and trainee pharmacists in elderly care.
3. Reviewing adverse incident reports and educating medical and nursing teams on pharmaceutical issues.
4. Reviewing expenditure reports and developing cost improvement initiatives.

Person Specification
Essential Qualifications
* Member of the General Pharmaceutical Council (GPhC)
* Postgraduate Clinical Pharmacy Diploma / MSc or equivalent qualification
* Independent Prescriber with appropriate annotation on GPhC register or willing to undertake
* Member of the Royal Pharmaceutical Society (RPS)
* MSc in Clinical Pharmacy or equivalent
* Formal management and/or leadership qualification
Knowledge, Skills, Training and Experience
* Comprehensive experience as a clinical pharmacist practitioner including a solid grounding in a broad range of clinical specialties
* Experience working as a clinical pharmacist in a hospital setting
* Commitment and evidence of continuous professional development (CPD)
* Computer literate and able to use Windows applications including Microsoft Word, Excel, Teams, and internet
* High standard of oral and written (including report writing) communication skills with attention to detail
* Able to demonstrate good leadership skills
* Good and broad understanding of current hospital pharmacy practice and future direction of travel
* IT skills and knowledge of the digital medicines agenda.
* Clinical pharmacy experience within elderly care and an enthusiasm to develop this
* Evidence of involvement in pharmacy practice research and/or audit
* Publication of research in a peer-reviewed journal
