Principle duties to include: Communication / Relationship Skills To actively participate in multi-disciplinary meetings. To support effective communication both internally and externally. To evaluate, assess and monitor clients progress in relation to their goals and care plans and to record accurately then report back any changes or concerns to an appropriate staff member. To empower clients, families and carers by ensuring that they have an understanding of the care and treatment offered. To provide an advocacy role by supporting clients, families and carers in articulating their concerns or needs. To maintain accurate documentation as required by the RCT in a clear and timely way. Communicate effectively with patients and carers, recognising individual needs and utilising a range of agreed communications e.g. electronic mail (email), SMS messaging (texting). Maintain confidentiality at all times, as required by legislation and trust policy. Analytical / Judgmental Skills Work as an effective and responsible team member, supporting others by demonstrating good practice including utilising mechanisms to develop and implement new ways of working including using information technology where appropriate. Participate in team activities that create opportunities to improve patient care. Adhere to the organisations policy for handling, recording and safe keeping of patients property. Work under the direction of the registered clinical practitioner and assist in the implementation of care according to the individualised care plan and contribute to the development and review of the care plans. Liaising with clients, colleagues, family and other professionals in ensuring that integrated quality care is provided. This will include assisting clients to learn new skills that promote independence. To empower clients and adopt a problem solving approach by encouraging and giving clients the confidence and skills to become independent. To provide a point of contact for patients and medical / clinical staff, ensuring that client concerns are answered with skill, sensitivity and expediency. To support carers in their role of carer. This may include communication and liaison with other agencies, including partnership agencies. To provide practical / personal support for people identified as having complex needs and sign post to the relevant health or social services. To support clients with their personal and emotional care needs, whilst respecting their dignity and maximising their independence. To collate monthly activity data and produce charts in Excel in order to support quarterly reports, liaising with the information department to ensure correct data is collected. Patient / client care To work closely with the RCT to contribute to an efficient service for patients with chronic respiratory conditions. To be able to carry out assigned tasks involving direct patient care and with appropriate training to also include: o Taking of specimens, including sputum and others as identified by the specialist nurse/physio o Routine observations of heart rate, blood pressure, temperature and respiration rate o Use of pulse oximetry o Undertake oxygen assessments, under direction o Participate in the running of pulmonary rehabilitation programmes Tasks to be carried out strictly within the care plans / guidance set by the specialist nurse/physio and any changes / concerns / problems to be reported back immediately. Policy and service development Contribute to service developments to improve practice and health care outcomes that are underpinned by local guidance, patient and carer needs and utilisation of multidisciplinary skills to formulate and test new developments. Develop and implement evidence based clinical practice within the service, developing specialist pathways. Contribute to measuring the effectiveness and productivity of the RCT through audits and the collection of data for the team performance reports. Contribute to and participate in the review and development of policies, procedures and strategies in the interest of users, the team and the organisation. Lead with writing policies and procedures relating to Community respiratory care. Undertake root cause analysis following serious incidents and then produce and implement related learning and development plans. Participate in clinical networks locally to develop clinical links and help forge new networks across Surrey Heartlands and nationally. Contribute to the development and delivery of CSH Surrey objectives and assist in the delivery of Clinical Governance arrangements, performance management and working to agree quality standards. Financial and physical resources Ensure that all resources are used effectively, with the minimum of waste, making recommendations where it is evident that appropriate changes may improve efficiency. Ensure maintenance of technical, clinical and non-clinical equipment e.g. computer and printer equipment, and to respond to reports of defects. Contribute to effective and economic use of resources e.g. local recycling schemes Human resources Works following direction from registered HCP and within the limitations of the role, always promoting high quality care and developing own skills and knowledge where appropriate. Contribute effectively as a member of the multi-disciplinary team, building appropriate relationships. To actively participate and contribute to the supervision process. To actively participate in the induction / orientation of new staff and students. To utilise the educational packages to develop self and role in line with PDP. Information resources Ensure accurate and timely data entry to contribute to the provision and analysis of information to improve patient care. Take personal responsibility for safeguarding and ensuring the quality of information including complying with the requirements of the Data Protection Act 1998. Create and use records including electronic in a manner that complies with legislation, and organisational policies for record keeping. Understand own and others responsibility to the individual organisation regarding the Freedom of Information Act. Facilitate patients access to records, adhering to policy, legislation, best practice and professional guidance. Research and development Support the RCT in on-going collection of data for local and national audits to assess effectiveness and productivity of the service Freedom to act To work under the guidance of registered practitioner acknowledging own skills and boundaries, taking opportunities to develop self knowledge, contributing to effective team work and respond to the changing needs of the service.
