Job summary As a Community Children's Nurse (CCN), you will provide a high level of specialist care to sick children and young people with complex health needs in the community setting. As well as providing direct care will provide support to parents and families, which may include teaching of specific care required to maintain the child at home whenever it is safe to do so. It is also expected that the CCN will provide support to patients in their home known to the children's acute community team, and specialist nurse services, including completing wound reviews, and home intravenous antibiotics. You will also be expected to work in partnership and liaise with the multi-disciplinary/multi-agency team. Main duties of the job Clinical Skills:Complete the Postgraduate Diploma (PGDip) Specialist Community Nursing if this has not already been completed. To teach and assess families/carers to carry out specific nursing care for a variety of conditions. To be responsible for assessing, implementing, and evaluating individual children's care plans. Work in collaboration with parents/carers to provide care to children and young people in accordance with the child's plan of care. To liaise with all members of the multidisciplinary team when changes occur. To support the provision of evidence-based care for children within the community setting with the support and guidance of the senior nurses and the clinical nurse specialist team. Empower the children/young people, families and carers to provide the complex care required. To make informed clinical decisions, using specialist knowledge and skills.Maintain safe working practices when using equipment and when training others in the delivery of the child's care. Co-ordinate and act as a keyworker for complex packages of care. Utilise skills to effectively manage emotions in response to significant events e.g. diagnosis of a condition or end of life care.
