Working across organisational boundaries and as an integral part of the Surrey Downs Health and Care System, the integrated health and care team will be responsible for providing Urgent community Response, enhanced, short term health and care support for people who would otherwise need emergency hospital admission as well for as those medically stable to return home from an acute hospital but requiring enhanced short term support as one stage in their on-going care pathway. The Home First Service operates as a 7 day service.
The post-holder is an experienced nurse who, acting within their professional boundaries will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care including assessment and diagnostic skills, for patients within the Home First Service. The post-holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the Clinical Lead and ACPs and the whole team to meet the needs of patients, supporting the delivery of policy and procedures, providing nursing leadership.
Key Responsibilities:
1. To deliver expert, specialist service for adults with complex health and social needs who may benefit from, or who are under the care of, the Home First Service.
2. To coordinate a seamless service through the development of enhanced interdisciplinary team processes and communication, within the Home First Service and across the wider Surrey Downs Health and Care whole system.
3. To empower patients and carers to make informed decisions regarding their goals and outcomes and the care they will receive to support them to meet these.
4. To support the development and evaluation of clinical protocols and systems of inter-agency documentation to enhance both continuity and the standards of care.
5. To understand and apply the Mental Capacity Act in regard of mental capacity in decision making and appropriate application of Deprivation of Liberty requirements and working with other agencies within the Safeguarding framework.
6. To attend meetings as required including deputising for the Lead Nurse and Clinical Manager as requested.
7. To develop close links with the wider community to enhance both care for individual patients and the wider development of Epson Health and Care.
