Job Description:
The Advanced Nurse/Clinical Practitioner (ANP/ACP) utilises expert clinical knowledge to manage patients presenting with acute, undifferentiated problems in addition to those with chronic long-term conditions. Working within their defined scope of practice, the ANP/ACP works as an autonomous Practitioner to assess, diagnose and plan care for patients of all ages, including infants. The post holder would assess and treat minor injuries and minor illnesses and will prescribe medications where qualified to do so, and where clinically indicated. Working across Urgent Care Services, the ANP/ACP will manage acute presentations to the Walk-In Centre, as part of a team of autonomous Practitioners; in addition to using the clinical knowledge and skills to determine urgency and best pathway for patients presenting to the Emergency Departments (streaming).
Main Responsibilities:
1. To assess patients at point of entry into the health care system with undifferentiated undiagnosed problems.
2. To provide care within their skills set across the full range (infants, children, adults and elderly).
3. To assess and treat minor injuries and minor illnesses.
4. To take responsibility for making autonomous decisions in relation to patient care.
5. Using advanced assessment skills of history taking and clinical examination, make a diagnosis and initiate an appropriate care plan including any necessary diagnostic tests or referral.
6. To promote health and screening in relation to primary prevention. Gain an understanding of chronic disease management and access training as appropriate.
7. To prescribe within current parameters according to prescribing authority for extended and supplementary prescribing.
8. To participate in the development and implementation of service/CCG guidelines and protocols.
9. Provide expert nursing clinical advice to colleagues including mentoring of junior practitioners.
10. Work with nursing and medical teams in providing a proactive management of patients with chronic disease according to local strategies.
11. Attend training events to maintain professional development.
12. Ensure confidentiality is maintained at all times as per Caldicott requirements.
Advanced Physical Assessment and Consultation Skills
Level 7
Essential:
* Minor Illness – Level 7 Essential
* Non-Medical Prescribing – Level 7 Essential
* Minimum 3 years’ experience post level 7 qualifications Essential
Desirable:
* Paediatric Course – Level 7 Desirable
