Job overview An exciting opportunity has arisen within the Warrington Community Learning Disability Team for a Community Learning Disability Support Worker to support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities. The post holder will assist in the delivery of care plans and programmes by - implementing programmes and reporting outcomes, contributing to the recording of plans/programmes and providing therapeutic activity where agreed and appropriate. Acting to as a nominated worker for individual clients who have learning disabilities and may also have extreme challenging behaviour including physical and verbal aggression, changes in mental health, forensic needs, complex physical health needs. Facilitate and support the development of appropriate personal care and independence with activities of daily living. Support self-help, communication and social skills acquisition The service model is one of supporting people, whose behaviours challenge services, working closely with specialist practitioners and other health professionals to support this complex group of individuals to remain in their community setting and, where an appropriate admission has occurred, to be discharged promptly and safely in line with national Transforming Care strategy. Detailed job description and main responsibilities Under the direction of the qualified practitioner, participate in the delivery of planned care within both inpatient and community environments.
