Job Summary
South Tyneside and Sunderland Paediatric Bladder and Bowel Team have an exciting opportunity for a Registered Childrens Nurse with significant experience of working in a paediatric area, to join our team. Due to a recent structure change, this post now sits under the Paediatric directorate.
We are looking for a Band 6 clinician who is filled with passion to become part of our service. Someone who possesses excellent communication skills with an ambition to continually develop the service to improve the patient/family journey.
The post holder will be required to undertake holistic Tier 2 level continence assessments, and work with children and families to develop individualised treatment programmes to promote continence.
The post holder will hold clinics in both Sunderland and South Tyneside and deliver training programmes to STSFT staff.

Main duties of the job
Team working hours Monday - Friday 08:30 - 16:30.
Service covers South Tyneside and Sunderland area.
To adopt a philosophy of continued professional development in line with service provision, working within the scope of Specialist Nurse Role following local policies/protocols/guidelines to meet the needs of children and young people with continence problems.

Job Description
Job responsibilities
To support in the day-to-day management of an integrated evidence-based Community Paediatric Bladder and Bowel Service, with responsibility for the delivery of patient care optimising the cost-effective use of all resources.
To support coordination of paediatric Bladder and Bowel service provision within Sunderland and South Tyneside.
To manage a caseload for the children's bladder and bowel service working at Tier 2 level.
To work with paediatric colleagues in providing an accessible and multidisciplinary continence service.
Ensure a seamless service and help reduce inappropriate referrals to secondary care by working in collaboration with tier 2 paediatric staff.
To respond effectively to the child's physical, psychological and social needs by working alongside Secondary care Education, Children and Young People Service and Social Services.
To transform service delivery by enhancing collaborative working across hospital and community.
To meet the needs of all children and young people with continence problems (including children with learning difficulties and physical disabilities) in order to achieve complete continence or manage their condition discreetly and effectively if full control is not possible.
To work in appropriate care settings, within the scope of a specialist nurse role following local policies/protocols/guidelines.
To assess individual children's needs using a comprehensive bladder/bowel assessment, determining a plan of care and implementing holistic, research-based health care (including initiating investigations and prescribing medication when necessary).
Work in collaboration with tier 1/2/3/4 services in bowel and bladder care to provide high quality care which follows best practice and meets the NHS outcome framework domains (1, 2, 3, and 4).
Support to ensure the service meets NICE guidelines, Department of Health and NHS England recommendations.
Triage all referrals into tier 2 Paediatric Bladder and Bowel service ensuring that local guidelines are followed, and referral is to the appropriate professional.
To help manage the provision of NHS disposable containment products for children in Sunderland and South Tyneside.
To act as a resource of specialist Paediatric continence advice in Sunderland and South Tyneside by providing training and support to internal and external staff.
To facilitate and support the development of a communication network for Paediatric continence advice and information for staff.
To ensure systems and processes are in place to maintain patient safety.
To support the development of user involvement within the Paediatric Bladder and Bowel Service to improve the child/family journey.
Support transition of care into adult Bladder and Bowel services.
