Job Title: Health Care Assistant
Reports to: Nurse Managers / Clinical Quality Manager Belper PCN
Job Summary: To work as part of our team supporting the nursing and doctor teams. To be flexible and responsive to the health needs of the population. Provide a high standard of clinical care.
Training & Support: In-house training and external training will be provided.
Hours of Duty: Part time/Full time - as per contract of employment.
Clinical Duties:
1. The Primary Health Care Assistant role involves phlebotomy, in line with national guidelines and protocol.
2. Acts as an autonomous Health Care Assistant in allocated surgery clinical duties according to protocols.
3. Health promotion NHS Health Checks after completing the training to undertake NHS Health Checks at practice and PCN-run clinics.
4. Ensure that any follow-up is clearly communicated to patients.
5. Routine blood pressure monitoring (both surgery and home visits).
6. Height, weight, alcohol, smoking, other checks & calculating body mass index (BMI).
7. First part of Long-Term Condition reviews which may include Height, weight, BP, Oxygen Saturation levels, working out BMI, and undertaking Diabetic Foot Checks, once training has been completed.
8. Recording and acting upon CVD and QRISK assessments.
9. Perform ECGs (both surgery and home visits).
10. Recording and downloading ECGs for both routine and emergencies.
11. Chaperone duties may be required.
12. Blood pressure recordings as per PCN / National guidelines (NICE), identifying new hypertension, Atrial Fibrillation.
13. Assisting with Minor Surgery.
14. Assist with fitting and removal of LARCs.
15. Removal of stitches and wound staples.
16. Wound Care.
17. Assist in vaccination campaigns, including influenza immunisation utilising Patient Specific Directions (PSDs) as per local policy/procedure.
18. Recording of all clinical/relevant health information collected, into patients electronic medical record (EMR) and reporting of abnormal clinical findings to a clinician.
Non-Clinical Duties:
1. Maintenance and stock control of consulting rooms.
2. Ensure cold chain policy is adhered to and documentation maintained.
3. Ensure clinical waste protocol is implemented.
4. Ensure sharps bins are appropriately assembled, labeled, prepared, and ready for collection by the local courier service.
5. Adheres to local Infection Prevention Control guidelines when handling samples and taking blood tests from patients.
6. Undertake any other duties, which may reasonably be required.
Confidentiality: Ensure that the PCN confidentiality policy is adhered to and follows Clinical Governance and the Caldicott principles. Information relating to patients, carers, colleagues, other healthcare workers, or the business of the PCN may only be divulged to authorised persons under the PCN policies and procedures relating to confidentiality and the protection of personal and sensitive data.
Administration and Professional Responsibilities:
1. Participate in the administrative and professional responsibilities of the PCN team.
2. Ensure accurate and legible notes of all consultations and treatments are recorded in the patient's electronic notes.
3. Ensure the clinical computer system is kept up to date with accurate details recorded, and templates are used to ensure quality standards.
4. Ensure accurate completion of all necessary documentation associated with patient healthcare and registration within the PCN.
5. Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
6. Attend and participate in meetings as required, including Quest sessions which will be compulsory to attend.
7. Ensure all Mandatory Training is completed as per our training policy matrix.
8. Be aware of our mission statement and act as a role model for Belper PCN.
9. Create and send recall letters for specific clinics as agreed with Line Manager.
10. Assist in seasonal and special projects as requested e.g. flu campaign.
11. Ordering and display of Health Promotion materials.
Management of Appointment System:
1. Ensure total familiarity with the appointment system including regular and incidental variations.
2. Book appointments and recalls ensuring sufficient information is recorded to enable retrieval of the medical record.
3. Monitor flow of patients into the waiting room ensuring the appointment system accurately reflects the arrival of patients.
4. Monitor effectiveness of the system and report any problems or variations to the Senior Receptionist or your Line Manager.
