Our client, an established Adult & Community Services organisation, is seeking a dedicated and compassionate Social Care Navigator to join their team. As a Social Care Navigator, you will play a vital role in supporting individuals in accessing appropriate and timely support services, ensuring their needs are met effectively. You will collaborate with health professionals, local organisations, and our client's team to facilitate seamless client discharges and provide initial advice and support to service users.
Responsibilities:
Conduct routine assessments and assist social workers with more complex assessments
Offer guidance to service users in navigating through local care and support services
Help service users identify and utilise available resources, including benefits and informal support
Complete social care assessments and reviews, ensuring service users are involved in decision-making processes
Identify eligibility for services in adherence to Care Act 2014 requirements
Assist service users and their relatives with financial assessments
Create support plans within available resources, highlighting concerns and unmet needs promptly
Provide information and advocacy to support informed decision-making by service users and carers
Collaborate with Commissioning colleagues to stay informed of available local provisions and make appropriate recommendations
Foster relationships with health-related agencies, the third sector, and local housing providers
Support early intervention to reduce the need for care and support
Conduct carers' assessments and reviews
Identify and raise safeguarding concerns according to local safeguarding procedures
Maintain accurate and complete records on our client's electronic systems
Participate in supervision, performance management processes, and team meetings
Continuously develop professionally through required training and champion new aspects of service provision
Work towards agreed targets and standards, monitoring and achieving personal targets
Collaborate with partner agencies, professionals, carers, and families to facilitate access to community services and resources
Contribute to multidisciplinary assessments, such as Continuing Healthcare assessments
Be available to work weekends as requiredRequirements:
Willingness to undertake continuous professional development or training
Experience in a social care or health environment
Ability to identify and raise safeguarding concerns
Good communication skills and experience liaising with professionals, agencies, and organisations
Proficiency in using IT tools, including Microsoft Word, Excel, and databases
Basic understanding of the role of resources in promoting independence and the process of self-directed support/personalization
Knowledge of eligibility criteria related to the Care Act 2014
Familiarity with ASC policies and procedures
Ability to establish and maintain effective working relationships with colleagues, partner agencies, service users, and carers
Experience in promoting fair access to services and working with diverse communities
Strong written and verbal communication skills, with the ability to explain complex and contentious issues concisely and clearly
Commitment to equality, diversity, and the principles of the workplace
Ability to manage a demanding workload and meet deadlines
Availability to attend various locations and evening meetings as required
