This is a post in primary care within the community mental health team in Denbigh.
We provide assessments from numerous agencies who refer through our SPOAA daily meetings, and offer brief interventions within groups and occasionally on a one to one basis.
A good knowledge of third sector agencies is useful as we work hard to become more familiar and signpost to these very helpful agencies, improving our contact with them and continually enhancing our working relationships.
Main duties of the job
An exciting opportunity has arisen in Primary Mental Health Support services for an experienced, motivated and forward-thinking practitioner to join a busy supportive team based in the Denbighshire area.
The successful candidate, while working closely with a multi-disciplinary team, will receive referrals directly from GPs and provide short-term interventions for individuals with mild to moderate illness. They will work closely with colleagues from secondary care and specialist services as well as third sector organizations and statutory agencies.
The successful candidate will manage a caseload of patients, working alongside them to formulate risk management plans and care planning based on robust assessment of need. We are looking for a good team player who has excellent communication skills and the ability to prioritize and manage their time effectively.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
