Job summary The Calderdale Community Rehabilitation Team provides assessment, treatment and advice to adults with a rehabilitation need and goal, with staff and service users working together to achieve patient-centred goals. The service comprises 3 geographically based teams within the Calderdale area. The role will support qualified allied health professionals to deliver a high quality therapy service within Calderdale community. The service supports patients who may have complex neurological conditions such as Motor Neurone Disease and Stroke, acquired brain injury, long term neuro-degenerative conditions, patients with acute and chronic musculoskeletal problems, fallers, frailty, orthopaedic conditions including post-surgery, and complex multi pathology conditions which may include patients with dementia or learning disabilities. The Community Rehabilitation Team is a multi-disciplinary team, and consists of Physiotherapy, Occupational Therapy, Speech and Language Therapy, Dietetics, Falls Prevention Workers and others support staff. The different disciplines work together in an integrated manner to provide the best treatment for patients. Main duties of the job The Post holder will: Undertake highly skilled, delegated, rehab support work. Work as part of the multi-disciplinary team, and will work under the instruction of our qualified Occupational Therapy, Physiotherapy, Dietitian and Speech and Language Therapy clinicians. Work to agreed competencies and protocols and guided by the registered practitioners, the post holder will be key in support of the treatment and care of patients and also their carer/relatives where relevant. Be working under indirect supervision of the registered practitioners in the team. Undertake delegated work following a rehabilitation plan devised by the registered practitioner or Assistant Practitioner Work as part of the wider Health and Social Care Team linking with staff from the hospital, Primary and Social Care. This may involve providing support to other community therapy services to support capacity and demand. The post holder will be required to undertake lone working in line with Trust policy. Administrative duties and general patient care across departments are required.
