We are looking for an organised, efficient and hard working individual to join our team as a care coordinator working from our offices in Kingston Upon Thames.
Clarendon Home Care is a door-to-door home care provider which services older people in the Surrey area.
As a care coordinator you will be the first line of support to our team of home care staff and will also field phone calls from clients, their families and other care professionals such as local authority social care teams, GPs, District Nurses, etc.
You'll have a professional telephone manner, great organisational skills and exceptional people skills. You'll have a passion for working in care and will be familiar with the Microsoft Office suite as well as some knowledge of the care sector. You'll work well in a team setting and you'll possess strong problem-solving and common sense skills.
Applicants must:
* Have an excellent telephone manner, attention to detail and the ability to work under pressure
* Display excellent communication skills, both written and verbal
* Be flexible in their work
* Be prepared to work evening shifts and some weekends
* Be able to liaise effectively with other health care professionals as well as with our clients and their families.
* Having a caring nature and a desire to want to support and help elderly people
* Be professional at all times
* Have a strong, approachable personality and first rate inter-personal skills
The responsibilities will include but are not limited to:
* On-call duties (some evenings and weekends)
* Updating and maintaining database records
* Liaising with clients, care workers and other care professionals on the phone and via e-mail
* Organising rotas for Home Care Assistants
* Call and task monitoring follow ups
* Acting as the first point of contact for care workers and clients
* Finding cover for care shifts
A DBS check will be required.
