Due to rapid, continued growth of our patient registrations, an exciting opportunity has arisen for an experienced, committed Advanced Nurse Practitioner to join Pen-Y-Bont surgery.
This role will involve working alongside a multidisciplinary team, including 4 GP Partners, a Prescribing Nurse, Practice Nurse, and Healthcare Assistant.
The successful ANP will work as an autonomous practitioner, diagnosing, managing, prescribing, and discharging patients with illness or injury across all age groups including young children. This may be by face-to-face consultation, telephone, or video consultation.
Main duties of the job
The successful post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
The successful applicant will undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home (if community ANPs are unable to attend), checking and signing repeat prescriptions, dealing with queries, and referrals.
Key Responsibilities:
1. Ten minute telephone consultations and fifteen minute face-to-face consultations
2. Share of home visits
3. Able to manage acute presentations
4. Able to manage chronic conditions
5. Able to order appropriate investigations (bloods/radiology)
6. Able to refer appropriately to secondary care or organize emergency admissions
7. Share of admin: clinical letters which require actioning/filing
8. Able to file pathology results
9. Able to see all age groups
10. No restrictions on prescribing
11. Able to see pregnant women in regards to non-pregnancy related issues
12. Able to deal with mental health presentations
Hours - Part-time / negotiable. This can be confirmed during the interview.

The postholder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
