Working hours- Monday- Friday 8:30- 4:30. Main duties of the job The post holder will be a senior and experienced Practitioner working within a Community Nursing Setting. The post holder will be responsible for the clinical leadership, operational management, and clinical development of the Community Nursing teams, including Community Matrons, and case coordination within the community nursing locality. The post holder will lead the development of the Community Nurses and Community Matrons and willfacilitate joint working between them to support the integration agenda and ensure that the service provided is of high quality, responsive to local needs, and demonstrates value for money. The post holder will ensure that the services are effective and well-coordinated, meeting the requirements of all relevant service specifications whilst keeping in line with CQC requirements. In addition, the post holder will work closely with the Service Lead, Advanced Community Nurse Practitioner, and Head of Community Nursing Services to develop the strategic direction in accordance with changing national guidelines and local agendas.
