This dynamic and innovative nursing role will focus on coordinating care, providing brokerage, and supporting admission avoidance strategies. As part of the role, you will offer clinical leadership to the nursing teams across Urgent Care - Emergency Department, SDEC (Same Day Emergency Care) and OPAU (Older Persons Assessment Unit), ensuring exceptional care for older patients with frailty and complex needs. Main duties of the job We are looking for someone who is highly organised, pro-active and thrives in a busy, challenging environment. Your main role as a Band 6 is to co-ordinate the admissions, discharges and transfers of our older patients through Urgent Care whilst maintaining the safety of patients throughout this process. This is a challenging but rewarding role and the applicant must have excellent communication and problem-solving skills and have the ability to adapt to changing situations when under pressure. The successful candidate will be instrumental in supporting the Urgent Care staff with the continuous improvement of the area, effectively monitoring and maintaining excellent standards of care that are reflected by patient experience. Sound clinical knowledge and experience in an acute setting is essential as you will be expected to support and mentor the more junior nursing workforce. The successful candidate must demonstrate expertise in assessing and implementing appropriate care efficiently for older adults who are both current inpatients and new admissions within the fast paced environment of the emergency setting.
