An excellent opportunity has arisen to join the Short-Term Triage Team as a Team Lead. This is a challenging role in a varied team dealing with a high volume of referrals daily and competing demands of the role. The individual will need to be motivated and have an interest in developing the role and joining us in taking the team through transformation and service improvements by being involved in strategic changes and implementation.
Main duties of the job
* To ensure that support/care packages promote independence and choice and offer the most cost-effective and beneficial outcomes, delivered and monitored.
* To provide professional supervision to an agreed number of social work staff, including completing the relevant appraisal process.
* To ensure own and staff compliance with legal responsibilities and policy guidance on equality and diversity matters, and human rights and to promote anti-racist and anti-discriminatory practice as good practice within the team.
* Assessments are undertaken within a multi-disciplinary setting, reflecting the perspective of the individual, their carer, their lives and aspirations; to encourage and support staff to work with a strengths-based approach as directed by Torbay and South Devon NHS Foundation Trust.
* To ensure assessment, care planning, and the subsequent commissioning of services are undertaken in a timely manner to ensure the right support is available in the most suitable environment, in the most cost-effective manner.
* To support and work closely with the Deputy CSM/Lead for Front End Service.
* To support and work cohesively with all social work leads and senior social work practitioners across complex care services and joint working complex cases with other social work staff.
* The post holder will be expected to take a lead role in Provider of Concern or Whole Service inquiries when required.
* To deputise for the Deputy CSM/Lead for Front End Service with all duties.

Person Specification
Qualifications and training
* Relevant professional Social Work qualification - Degree/Diploma in Social Work, Certificate of Qualification in Social Work or Certificate in Social Services.
* PQ1 and commitment to post-qualification training.
* Professional knowledge acquired through degree/diploma in social care, supplemented by further specialist knowledge, for example in needs of a specific client group, experience to masters equivalent level.
* Any preferable requirements which could be obtained once in post.
Knowledge and experience
* Ability to present an excellent role model for good practice for social care staff within the multi-disciplinary team.
* Ability to act as a consultant on practice-based issues for other social care practitioners.
* Ability to take a lead role in jointly worked complex cases.
* Ability to think laterally within Departmental eligibility criteria, whilst maximising quality outcomes from resources.
* Ability to work on own initiative while making positive use of supervision and training.
* Ability to use IT-based Care management systems.
* Ability to make decisions in accordance with the statutory requirements of the post.
* Ability to manage time effectively in assessing, planning, monitoring and reviewing clients' needs and services.
* Good reporting and recording skills.
* Working knowledge of electronic communication systems.
* Ability to communicate verbally and in writing with a broad range of people.
* Commitment to the concepts of a needs-led service, quality standards and service user choice within the context of Best Value.
* Working in the legislative framework which includes knowledge of the Mental Capacity Act 2005 and The Care Act 2014.
* Good knowledge of other Health & Social care legislation.

* Highly developed specialist knowledge underpinned by theory and experience.

Specific Skills
* High level Care Management experience.
* Adult Protection Investigator's training and experience.

* Current registration with the General Social Care Council.
