Managed a caseload of adults with complex mental health needs, providing assessments, support, and interventions to promote recovery and well-being.
Conducted comprehensive mental health and social care assessments under the Care Act 2014 to identify needs and develop personalised support plans.
Worked in collaboration with psychiatrists, psychologists, community nurses, and other professionals to ensure a multi-disciplinary approach to care.
Undertook risk assessments and risk management planning to safeguard individuals and others, responding promptly to crisis situations.
Provided support and advocacy to service users, ensuring their voices were heard in care planning and decision-making processes.
Facilitated access to appropriate community resources, housing, employment support, and therapeutic interventions to enhance independence and quality of life.
Prepared reports and participated in Mental Health Act assessments, contributing to decision-making around hospital admissions, community treatment orders, and safeguarding measures.
Attended and contributed to multi-agency meetings, including case conferences, safeguarding reviews, and care planning discussions.
Ensured accurate and timely record-keeping, maintaining compliance with legal and organisational policies.
Supported families and carers, offering guidance, referrals, and interventions to help them manage the challenges associated with mental health conditions.Requirements:
Social Work England registered
Social Work Degree or equivalent
