The post holder will work at all times under the delegation of a registered practitioner in the delivery of person centred care. The post holder will be responsible for following care plans and recording all personally generated observations and documenting care given in individual's notes. The post holder will assist in the delivery of therapeutic interventions and activities to enhance the patients rehabilitation and recovery and discharge pathway. The post holder will be expected to undertake flexible working hours, inclusive of unsocial hours and weekends.
Main duties of the job
1. Assist the Registered Practitioner in the assessment, planning, treatment, management and evaluation of care.
2. Escorting the patient into the community following their care plan and rehabilitation and recovery needs.
3. Assists patients to achieve physical and emotional wellbeing and comfort encouraging independence where appropriate.
4. Supports patients to access local community via public transport and Health Board vehicles when appropriate.
5. Respects the privacy, dignity, needs, beliefs, choices and preferences of patients and carers.

Person Specification
Qualifications
* Minimum of Qualification Credit Framework - QCF Level 2 (formally NVQ Level 2) in Health or has the equivalent knowledge, skills and experience.
Experience
* Experience of working/caring in a health or social care environment.
* Evidence of maintaining to undertake further learning specific to the role & personal development.
* Demonstrate an understanding of the scope of the role within a team.
Knowledge
* Knowledge and understanding of the Code of Conduct for Healthcare Support Workers in Wales.
* Fundamental aspects of care and the importance of the promotion of health and wellbeing.
* The principles of confidentiality and the principles of consent prior to undertaking care and the principles of Safeguarding.
* Understanding of when to seek advice and escalate to the appropriate registered professional for expert help and advice.
