Requirements:
* To work within the context of the Care Act 2014, to provide person-centred Social Work intervention, Strength-based work, preventive approach and enabling service users and carers to optimise and maintain their independence and wellbeing.
* Work with clients to produce a reablement plan which will help service users identify and achieve their goals relating to a range of social care tasks such as washing and dressing, showering, meal preparation and helping people engage in community activities.
* To be able to carry out joint working with therapists and to have knowledge of assistive technology.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
