Your Impact:
We are seeking a Young Carers Practitioner who will offer support to young carers, working with families to make a huge impact and support the young people to ensure they have the same life opportunities as their peers. Family Action’s Young Carers Service in Rotherham provides support to Young Carers, Young Adult Carers, and their families. Family Action has delivered the Young Carers service since 2011 and is an established provider within the sector. We operate a whole family approach to working, with the child, young people or young adults being at the center of the support.
Main Responsibilities:
1. Taking a lead in completing a Young Carers Needs Assessment or Young Adult Carers Assessment which will inform a package of support tailored to the young person and their family’s specific needs.
2. Support delivered through one-to-one sessions, home visits, school, college or university visits, telephone support and small peer support sessions with other local young carers and young adult carers.
3. Deliver age-appropriate focused events and support groups to provide peer support and learning with partner agencies.
4. Operate a whole family approach at all times, as well as supporting and signposting to other family members.
Main Requirements:
1. Enthusiasm, a creative approach to work and an ability to engage positively with a wide range of people and ages.
2. A sound knowledge of child/young adult development, safeguarding, and the assessment process is essential as is sharing our commitment to co-production.
3. Qualified to a level 3 or above with a Recognised professional qualification in social work, health, education or equivalent.
4. Excellent communication skills.
5. Experience in lead professional roles is desirable.
6. Appointments are subject to Family Action receiving a satisfactory disclosure from the Disclosure and Barring Service.
