About the Team As a Community Nurse you will work alongside paid and family carers to support people with learning disabilities to access mainstream health services by supporting clinicians to make reasonable adjustments. Your skills in adapting and creating the right approach for the individual will help bridge the gaps and reduce the risk of avoidable health conditions. About the Role You will develop relationships with wider health partners to address the health inequalities for people with learning disabilities identified through the LeDeR Reports and increase the uptake of health initiatives by engaging people with learning disabilities and their carers. You will have access to a range of training and on the job training to enable you to develop as a practitioner and take part in projects with partners such as NHSE. About you Essential criteria: Qualified Registered Nurse Full UK Driving Licence and access to own car Some Learning disability experience Ability to travel to alternative sites within reasonable timescales Multi-disciplinary Team working experience Able to cite evidence-based practice Desirable criteria: Community and hospital-based health care experience Knowledge of social model of disability Teaching skills This is an ongoing role and applications will be considered as and when received Benefits of working for us How to Apply As part of your application, please upload your most recent CV.
