Job summary An exciting opportunity has arisen for a Diabetes Specialist nurse to work within our Diabetes specialist nurse team at South Tyneside and Sunderland NHS Foundation Trust. The diabetes specialist nurse will work within a well-established diabetes specialist nursing team delivering patient care, leading the management and care delivery for patients diagnosed with Diabetes. The Diabetes specialist nurse role will consist of providing expert clinical knowledge to patients with Diabetes and supporting those patients who are newly diagnosed Diabetes. There will be a requirement to contribute to the ongoing development and delivery of nurse lead service for patients. Main duties of the job Ensure that safe, effective and holistic patient care is delivered, and coordinate patients overall plan of care throughout the patient journey.
