Job Title: Social Worker – Older Adults/Hospital Team
Location : Conwy County Borough Council
Rate : £35 per hour
An exciting job role has recently become available at the Conwy County Borough Council. They are looking for a new member for their Older Adults/Hospital team.
What will your responsibilities be?
In this role, you will assess the mental status, cognitive skills, and health issues of older adults. You will organize and educate patients about their medications, provide information on personal safety, disease prevention, and lifestyle adjustments, and link patients with community resources while managing care plans. Additionally, you will need to work effectively within a multidisciplinary team.

Qualifications and Experiences:
To be successful in this role you must have Social Care Wales registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
