Job summary A new and exciting opportunity has arisen for skilled health care support workers to be a part of the development of a redesigned Nursing Care Model with the Hillingdon Care Connection teams. The Care connection teams comprise of a Guided Care Matron, GP, Health care support workers, an administrator and well being and support assistants- Together they provide active case managements for adult patients with long term conditions in order to prevent unnecessary hospital admissions. The successful applicant may have contact with patients or service users. Main duties of the job Please note community staff travel considerable distances through out the day. All applicants will need to be able to drive and have access to a car. The role of the Health care support worker will be to support the team with undertaking clinical activities such as the following (List not exhaustive) Home visits for clinical tasks Joint visits with matrons when required Observations and NEWS 2 Occasional medication monitoring- one week maximum. Phlebotomy Simple wound care Eye drops - short term.
