ChippenhamCommunity Team has an exciting opportunity for a part time Band 6 SpecialistCommunity Occupational Therapist to join our team. If you are passionate aboutdelivering community-based therapy,improving quality of life for individuals, and are committed to rehabilitationin a home environment, we would love to hear from you. The post is for hours per week.
Our multi-disciplinary team is based in ChippenhamCommunity Hospital and provides complex interventions to a wide range ofpatients in their own homes and other community clinical settings. Our therapy team consists of 6 therapy staff whomanage patients from 5 local GP practices. Our therapy service is providedbetween the hours of 08:00 and 18:00 including some weekends but this isflexible. We also work closely with our wider partnersincluding Specialist Services, Social Care, Acute Hospitals and Primary Care.
You will be working within a friendly and supportive team andthe role includes a focus on rehabilitation, long term condition management and supporting hospital discharges. You will be required to hold a complex clinical caseload and provide occupational therapy assessments to identify goals, formulate and deliver individual treatment programmes and order a wide range of equipment.
Main duties of the job
The role also includes responding to urgent referrals, liaising witha wide range of colleagues to plan supported discharges, supporting other staffmembers and being involved in ongoing processes to provide a continualproactive and effective service.
This is an idealopportunity for an experienced Occupational Therapist keen to develop enhancedskills and knowledge as part of a dynamic, multidisciplinary team. You willneed to have good organisational and IT skills combined with experience ofworking with adult and older persons services.Excellent clinical and communication skills and the ability to work underpressure are also required. We are looking for someone who is enthusiastic and whocan work flexibly and is willing to contribute to the ongoing development ofthe services we provide.

Qualifications

12. Dip COT/BSc/BSc(Hons)/MSc OT

16. To possess a current, valid driving licence and to be a competent driver
17. Evidence of continuing professional development
