Description Salary £35,235 - £39,513 Per annum | Permanent, Full Time | 37 hours per week | Community North | Wyre - Thornton, Lancashire, About the Role: Lancashire County Council is looking for a committed and professional Social Worker to join our Adult Social Care team. This role involves providing a strength-based, outcome-focused assessment service to adults across community and hospital settings. You will be responsible for delivering high-quality social work services, promoting independence, enabling recovery, and safeguarding adults. Key Responsibilities: Deliver a professional social work service in line with statutory responsibilities and organizational policies. Contribute to service delivery that is strengths-based and outcome-focused. Collaborate with other professionals and agencies to address complex needs and achieve ideal outcomes for individuals and their carers. Provide high-quality assessments and support plans in consultation with individuals and their carers. Complete written reports and present information effectively. Work with health colleagues to ensure safe and timely discharges from hospital. Promote the creative and flexible use of personal budgets. Communicate effectively with people, carers, other agencies, and professionals. Manage and prioritize a caseload efficiently. Maintain accurate records using electronic systems. Support and mentor newly qualified or less experienced staff and students. Undertake continuous professional development and mandatory training. Maintain emotional resilience and professional wellbeing. Participate in a duty system and respond to urgent situations as necessary.
