This is an exciting opportunity for an Occupational Therapist wanting to specialise in Frailty to join our frailty team as a static member. The team covers the Emergency Department, Rapid Assess Clinic, Frailty Assessment Unit, Rockwood Short Stay Unit and Frailty outliers. It's an exciting time to be part of frailty services with local and national developments recognising the importance of preventing hospital admission, hospital optimisation and discharging this cohort of patients back home in a timely manner.
Job Summary
To be a positive and proactive member of the frailty team. To assess patients holistically in a timely manner using a frailty assessment based on the Comprehensive Geriatric Assessment. To provide effective and efficient therapy input to meet patients' needs. To refer patients to the community to facilitate safe discharges, ensuring the correct community support to meet their needs. To work effectively in the multidisciplinary team for positive patient outcomes.
Main duties of the job
Main tasks and responsibilities
Clinical
1. For physio applicants, to be part of the on-call respiratory rota and weekends.
2. For all applicants to be part of the 7-day service once established at the Trust.
3. To provide skilled therapy interventions across the frailty service ED / FAU, RACE, Short Stay Unit and cover frailty outliers as required.
4. To be part of the wider Older Adults Therapy Team, providing cover across older adults when required and engaged in the Inpatient Therapy Department service.
5. To be professionally and legally responsible and accountable for all aspects of your own work/caseload.
6. Use resources effectively to manage competing priorities and demands.
7. To work in a blended capacity to complete holistic frailty assessments.
8. To plan and implement interventions, in collaboration with patients, setting joint goals with the multidisciplinary teams and patients underpinned by clinically effective treatment/management programmes.
9. To provide accurate and effective handover of patients to partner agencies and community services to achieve a seamless transition of care for patients.
10. To seek advice from colleagues if patient presentation is outside of clinical competence.

Person Specification
* Degree in Occupational Therapy
* HCPC registered
* NHS experience of working in a variety of relevant clinical areas (Emergency Department, AMU, Acute wards), for physios respiratory competent
* Evidence of Continuing Professional Development
* Working with acutely unwell, older adults in an inpatient setting
* Supervisory experience with support staff and/or students
* Effective written and oral communication skills
* Effective clinical reasoning skills
* Clinical educators' course
* Experience of research, audit and outcome measures.
* Working in an OT and PT blended capacity
* Frailty
