As an Assistant Practitioner, you will be the first point of contact for people reaching out to our Learning Disabilities Service. You will:
* Manage a busy inbox, respond to phone queries, and welcome visitors to the office.
* Provide timely advice, guidance, and signposting to residents.
* Support social workers by implementing support plans and helping individuals achieve their identified goals.
* Conduct strength-based reviews, update outcome-focused support plans, and participate in safeguarding enquiries under the supervision of a senior practitioner.
* Assist carers by completing assessments to ensure they receive the support they need.
* Maintain detailed case records, ensuring accurate and timely documentation.
This role requires you to be office-based Monday to Friday (9am – 5pm) to ensure consistent support for residents and collaboration with colleagues.
About You
We are seeking a dedicated and resilient individual with a strong background in social care or a related field. You will have:
* The ability to build trust with service users and their families.
* Strong communication skills to manage complex conversations and provide clear advice.
* Experience in completing assessments, developing support plans, and delivering focused interventions.
* A proactive and organised approach, with the ability to manage competing demands.
* Knowledge of safeguarding practices and the confidence to escalate concerns appropriately.
* A commitment to promoting independence, positive outcomes, and the use of assistive technology.
