Albert Street provides supported accommodation for individuals over 18, who are homeless and with co-occurring mental health/ substance misuse issues. You could be working with these incredible individuals to teach them tenancy management skills and engage them with external services to really make a difference to their lives. What is great about the role? Albert Street provides person centred and tailored support to individuals accessing the service. We deliver support that meets the needs of the individuals, and which provides opportunity for them to achieve their goals and aspirations. Day to Day Duties: As a Complex Needs Project Worker you will support the Project Workers with facilitating person cantered support plans and work extensively with a service users to promote mental health recovery and seek treatment and recovery for substance misuse. Your day will consist of: Engaging and interacting with the people we support Writing and following a support plan with the individual you are supporting Delivering workshops relating to tenancy management and health and wellbeing Liaising with external agencies to arrange multi-disciplinary meetings Supporting with attending appointments with the people we support Welfare checking Addressing any additional needs, the people we support might have Our motivation is to ensure we equip these individuals with the knowledge and tools they need to live independently as well as re-engage them into the appropriate health services. This is a challenging role and you will be faced with complex behaviours and needs, including failing health, complex physical health, and substance misuse issues. You will work with a range of different personalities and behaviours so you will need to be able to adapt your approach to engage with a diverse cliental.
