Emergency Care Practitioner/ANP - Highbridge Medical Centre
Highbridge Medical Centre is currently recruiting for either an Emergency Care Practitioner/ANP to join their team on a permanent basis. The role available is for 35 hours per week with a salary of £40,221.81 - £51,465.14 per annum, pro rata, depending on experience. The position also benefits from an NHS or NEST pension.
The position is based at Highbridge Medical Centre, with cover at Burnham and Berrow Surgeries potentially required.
Interviews will take place the week commencing 21st October 2024.
Main Duties of the Job
The post holder will be an experienced paramedic acting within their professional boundary to provide care for the presenting patient. This role is expected to develop and evolve over time as the successful post holder gains experience.
The primary care practitioner will work closely with our GPs to advise treatment and healthcare for patients, including those presenting in the primary care setting and those in the community.
The post holder will attend home visits, including residential and nursing homes, where they will liaise with staff, family, and other stakeholders in the production of care plans to be shared with other providers and stakeholders.
The practitioner will also triage acute patients presenting in the surgery environment.
They will demonstrate safe clinical decision-making and expert care for patients.
They will work collaboratively with the multi-disciplinary team to meet the needs of our patients.

Job Responsibilities
* Conduct visits to patients in their own home or a care home environment.
* Assess patients with a range of acute, non-acute, and chronic medical conditions.
* Liaise with GPs to agree on diagnosis, any further investigations which may be needed, and appropriate treatment.
* Liaise with GPs to admit patients to secondary care if they are in acute medical need.
* Refer patients as appropriate to other members of the multidisciplinary team, secondary care, and other relevant care providers.
* Collaborate with GPs, nursing staff, and non-clinical support staff, using individual skills and experience to contribute positively to the practice and the development of a team approach.
* Support the delivery of new and improved primary care services to meet the changing needs of our practice population.
* Provide education and training to other staff as appropriate and encourage teamwork and the sharing of best practices.
* Help develop our practice for the overall benefit of our patients.
* Assess undifferentiated patients with a range of acute, non-acute, and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses, and liaise with GPs to agree on the appropriate follow-up treatment and care plans.
* Maintain accurate record keeping on our medical record system (EMIS Web).
* Undertake and interpret a range of diagnostic tests and routine clinical procedures, including but not exclusively ECG, venepuncture, and basic observation recording.
* Work within all relevant practice policies and procedural guidelines, e.g., infection control, chaperoning, risk management, health and safety.
* Assist in delivering specialized programmes of care and care packages.
* Contribute to the management of disease registers, e.g., End of Life Register.
Person Specification
Experience
* Qualified paramedic or specialist paramedic registered with the HCPC.
* A minimum of 5 years of paramedic work experience.
* Evidence of appropriate continuing professional development activity.
* Ability to assess and manage patient risk effectively and safely.
* Diploma or Degree in a healthcare-related discipline.
Qualifications
* GCSE or equivalent in English and Mathematics.
* A Level qualifications or equivalent.
* Evidence of continual professional development.
Knowledge & Skills
* Excellent interpersonal and communication skills.
* Knowledge of the primary care operating environment.
* Excellent analytical skills, including analysis of data.
* Time management skills and the ability to prioritize workload.
* Demonstrable commitment to and focus on quality and safety.

* Understanding of the South Somerset Symphony Programme and New Models of Care.
* Understanding of the NHS GP Contract.
* Use of EMIS searches and protocols.
* Able to multi-task and cope with an unpredictable work pattern.
* Able to work outside of normal hours as and when required.
