CHC Nurse Assessor – Hybrid Role – Birmingham

Service Care Solutions is recruiting for an experienced Continuing Healthcare (CHC) Nurse Assessor to join an NHS team on a hybrid contract based in Birmingham.
This is a temporary role offering a competitive daily rate of £320 Ltd, expected to fall outside IR35. A PAYE rate of £251.45 is also available.
Role Overview:
The successful candidate will support the Lead Complex Manager and Commissioners to ensure that CHC programmes are effectively planned, delivered, and reviewed. The role involves working across inter-agency boundaries, leading on complex cases, and ensuring robust safeguarding and legal compliance.
Key Responsibilities Include:
Coordinating timely quality assurance reviews for patients in specialist placements and ABI care.
Acting as a specialist practitioner, interpreting and implementing national guidance.
Leading on complex and sensitive cases, including representing the ICB in legal proceedings.
Facilitating and contributing to multi-disciplinary assessments and patient reviews.
Liaising with Local Authorities, other ICBs, and internal teams to ensure integrated service delivery.
Leading the implementation of Liberty Protection Safeguards for appropriate community patients.
Promoting and implementing Personal Health Budgets in collaboration with families and internal teams.
Representing the ICB in safeguarding processes, ensuring plans are reviewed and monitored effectively.
Supporting the Senior Clinical/Professional Lead with team business planning and performance reporting.
Requirements:
Must hold a clinical or professional qualification (e.g., NMC or HCPC registered).
Proven experience within CHC or a similar complex care environment.
Strong understanding of safeguarding, legal processes, and multidisciplinary working.
