Job summary An exciting opportunity has arisen for a Band 6 Community Practitioner to join our South Central Team within the Older Adults Services in Northumberland. Our Older Adult Services provide specialist mental health assessment and intervention for older persons with complex and enduring mental health problems, including those younger persons with an ongoing dementia type illness. The multi -disciplinary team comprises a highly skilled and experienced workforce of Consultant Psychiatrists, Nurses, Psychologists, Occupational Therapists and Physiotherapists. Main duties of the job We deliver a wide range of evidence-based interventions through clearly defined care pathways. We have developed a comprehensive supervision structure for staff including individual supervision and peer support through our clinical networks linked to our care pathways. We believe in the development of all our staff and encourage personal development. Our Community Practitioners are expected to undertake assessment, and implement evidence based treatment packages. They act as Care coordinator for complex cases involving multi-disciplinary working. They will be responsible for supervision of students and junior staff. They will also do duty on a rota bases.
