Have an awareness of the local and national drivers influencing Stroke Rehabilitation. To follow the RCOT and CSP Standards and Guidelines for Stroke, including all other guidance affecting Allied Health Professional services.
Receive information relating to patients' diagnosis, treatment and rehabilitation; act on any response within your scope of practice, and provide feedback to registered staff. Liaise with family members as required to meet the needs of the patients. Establish and maintain effective working relationships both within the Partnership Trust and with external organisations to meet service requirements and support the Trust's values.
Work with individuals and carers who may find it difficult to engage with the service or aspects of the service.
Be responsible for your own practice within your skills remit and competence as a Rehabilitation Assistant Practitioner. Implement treatment programmes autonomously to meet patients' needs under the supervision of a Registered Therapist, facilitating intervention, monitoring the individual's episode of care, and progressing this intervention as appropriate.
Identify SMART goals for patients on your allocated caseload and sign off goals that have been met. Be responsible for the safe and competent use of equipment by patients and staff. Recognise the importance of risk assessments as appropriate with Trust procedures.
Support non-complex patients through their hospital admission to meet service demand and be actively involved in discharge planning, including:
1. Goal setting meetings
2. Board rounds
3. Case conferences
4. Family meetings
5. Ordering equipment from Medequip
6. Completing referrals (e.g., community stroke team, wheelchair referrals, stroke association, olive branch)
7. Environmental risk assessments
Maintain accurate and comprehensive records in line with Trust standards of practice. Be responsible for your own personal and professional development, fulfilling post requirements in meeting lifelong learning and engaging in continuing professional development. Promote the involvement of patients and carers in the service.
Support and train band 3 therapy staff or other staff within the ward MDT in relation to areas within your scope of practice. Report issues with service provision/patient care to the line manager. Support with feedback of figures for the Stroke Specific National Audit Programme (SSNAP) to local data collectors, and support with any other ongoing audits.
Undertake any other duties required by Team/Clinical Lead and Specialist Services Manager which are commensurate with the post. Work within service model demands; ability to undertake bank holiday, weekend, and shift patterns with lone working as required.
Car driver is desirable to support with service demands and some community working, where appropriate.
