Clinical Pharmacist - Weald Primary Care Network
West Kent Primary Care (WKPC) is seeking to employ a Clinical Pharmacist on behalf of Weald Primary Care Network (PCN) to work across one or two of their member practices.
Interview date: scheduled for the week beginning Monday 28 April 2025.
Main duties of the job
The Practice-based Clinical Pharmacist will improve the quality and safety of care offered to patients of the Weald PCN. They will have regular contact with patients and will reduce the workload of GP prescribers in the practice. They will also support the management of patients with long-term conditions.
As part of this role, the PCN will fund for you to be enrolled into the NHS England CPPE Education Programme which lasts for 18 months. Study leave and time off to attend courses will be built into your work plan.

Job responsibilities
* Work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing.
* Perform face-to-face medication reviews of patients with polypharmacy, especially for those with frailty and/or with multiple co-morbidities and prescribe alternative medications.
* Undertake face-to-face reviews of patients with specific long-term conditions that fall within their competency (such as asthma reviews).
* Carry out Improved Access and Extended Hours sessions, including some weekend work.
* Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship.
* Contribute to practice financial stability through optimisation of medicines-related targets in QOF delivery.
Person Specification
Experience
* Previous experience as a community or primary care based pharmacist
* Experience in providing clinical advice and expertise on medicines to colleagues and patients
* Knowledge of structured medication reviews and producing recommendations for prescribing and monitoring
* Knowledge of medicines management at point of hospital discharge including patients living in care homes
* Experience of managing long-term conditions in a clinical setting
Qualifications
* University Degree in Pharmacy Sciences
* Registered with the General Pharmaceutical Council
* Registered Prescriber
