Job Title: Homeless Prevention Pathways Coordinator - 12 Month Fixed Term Contract
Reports to: Team Leader
Salary: Scale 6, Points 26-28, £26,229 - £27,982 per annum, pro rata
Hours of work: Full Time (37.5 hours per week)
Key Responsibilities:
* To receive referrals from Personal Advisors or Care leavers themselves and when necessary undertake holistic housing and support needs assessments.
* To facilitate access to the commissioned youth pathway accommodation and or support where possible.
* Support to access wider housing options within the exempt sector or commissioned specialist services such as Offenders, DV, Families, Mental Health and Learning Disabilities.
* To plan responses to presenting needs and circumstances in partnership with young people, parents/carers, other agencies and professionals in order to identify and negotiate the most suitable housing options.
* To ensure that placements/ services identified to meet the needs of young people referred take into account issues in respect of health, educational need, gender, ability, religion, ethnic origin, culture and linguistic background.
* To provide professional advice and support to a range of key stakeholders such as Social Workers, Heads of Service, Youth Offending Teams (YOTS) on a range of services/ provisions that are available to meet the needs of young people.
* To work closely with other organisations and agencies to provide holistic multi-agency responses to housing and support needs including private sector.
* To manage and prioritise own caseload and evaluate each case making decisions on appropriate options, meeting deadlines and referring to other services and outside agencies as appropriate.
* To attend designated meetings i.e. with Birmingham Childrens Trust, monthly accommodation provider meetings and case reviews regarding young people as and when required.
* To ensure that service is accessible to young people, all relevant parties kept abreast of outcomes and follow-up support provided as and when required.
* To keep up to date with current legislation, regulations and other new developments relating to young people and housing resources etc.
* To maintain timely recording and case planning in relation to case responsibilities to high and rigorous standards using electronic databases and client recording systems where available.
* To participate on a duty rota and lock up of building.
Strategic role:
* To assist in the service development and achievement of key performance indicators in line with the Service Level Agreement (SLA).
* To highlight trends, areas for development and gaps in service provision, to Line Manager and assist in developing action plans.
* To assist in updating policies and procedures.
* To maintain statistical information for monitoring and evaluation purposes in order to produce reports for key stakeholders as and when required.
* To liaise with key partners/funders as appropriate.
* To represent and promote the work of the agency and/or project at relevant external and internal meetings and forums.
* To deliver training and briefings as and when required.
* To be involved in the induction of new staff, to supervise and support volunteers, trainees and students.
* To monitor the performance of the service and provide reports and statistics as and when required by your Line Manager.
QUALIFICATIONS:
Essential:
* NVQ Level 3 (Advice & Guidance, Housing or equivalent or other relevant/equivalent qualifications)
ROLE EXPERIENCE:
Essential:
* Working with young people in the following settings/Types of work – Youth Work, Residential or Social Work, Probation, Community Work, Education and Training, Personnel Management.
Desirable:
* Working with volunteers/Students
* Supervision experience
KNOWLEDGE:
Essential:
* Knowledge of relevant legislation and housing rights.
* Ability to maintain, revise and improve standards.
* Knowledge of and commitment to Equality & Diversity, Anti Discriminatory practice.
Desirable:
* Personnel/management knowledge.
