An exciting opportunity has now arisen within the Manchester Macmillan Palliative Support Service for an Assistant Practitioner/Nurse Associate.
Working within the Macmillan Specialist Palliative Care Team, contributing to the provision of a co-ordinated and comprehensive service for patients receiving specialist palliative care, including patients resident in care homes in Central Manchester.
Responsibilities:
1. The post holder will work as part of the Macmillan Specialist Team multi-disciplinary team comprising of Medics, Dietician, Occupational Therapist, Physiotherapist, Nurses, and Speech and Language Therapist.
2. Provide effective support rehabilitation and/or palliative care to patients with a diagnosis of cancer at any stage of the patient’s illness from diagnosis onwards.
3. Assist other professionals in the implementation, delivery, and evaluation of rehabilitation and education programmes to promote independence and encourage improved health, functional capabilities, and quality of life.
4. Hold a small caseload under the supervision of the responsible clinician, supporting patients with life-limiting illness to maximize their health and enable them to be cared for in their preferred place.
5. Demonstrate the ability to be mobile within the community.
