We are currently recruiting an experienced full-time Care Coordinator.
We are steadfast in delivering high-quality care, ensuring optimal care standards in our communities, preventing illness, minimising the necessity for hospital services, and enhancing the management of chronic conditions.
In the pivotal role of a Care Coordinator, you become an integral link between our care workers, clients, Next of Kin (NOK), and professional counterparts. If you are a bright, bubbly individual who can expertly multi-task and deliver an exceptional customer experience, this role could be your ideal fit.
The successful Care Coordinator will have:
* Previous Care Coordination experience
* Excellent customer service skills
* Experience with planning software such as People Planner, CM2000, etc.
* The ability to work well under pressure, good time management skills, and initiative
* Good influencing skills – proven ability to convince others and gain their respect
* Excellent communication skills – reasonable telephone manner and the ability to build relationships, listen, and explain things clearly at all levels
* A willingness to participate in an on-call system for out-of-office hours on a rota basis
* An excellent working knowledge of Microsoft Office packages
Main duties -
* Manage the day-to-day operations of the care service, ensuring that the highest standards of care are provided to all patients
* Develop and maintain staff rotas to ensure adequate coverage for patient care, activities, and emergencies while optimising staff scheduling
* Monitor compliance with regulatory standards, policies, and procedures to ensure a safe and secure patient environment
* Collaborate with patients and their families to develop personalised care plans that address their unique needs, preferences, and goals
* Train staff and ensure that all staff are adequately trained and supervised to provide the best possible care for each patient
* Oversee the safe and accurate administration of medications, ensuring adherence to prescribed schedules and proper documentation
* Facilitate access to external support services, healthcare professionals, and specialists as needed to enhance patients' overall quality of life
