Band 6 Occupational Therapist - Early Supported Discharge Team - Full time 37.5hrs p/w. Maternity Cover
An exciting opportunity has arisen to recruit a Band 6 Occupational Therapist to join the Early Supported Discharge (ESD) Team at Salisbury Hospital for Maternity Leave cover. The ESD Team was formed in 2016 as a pilot scheme to support patients with an early transition home from hospital and to provide Occupational Therapy (OT) and Physiotherapy in their home environment. The initial focus was to enhance the speed and quality of discharges for hip fracture patients. The team has since evolved into a 'discharge to assess' therapy team and works alongside patients from: orthopaedic, stroke, elderly, medical and surgical wards. Due to the team's success, the ESD team is now a well-established and evolving team within the hospital.
The team works both within the acute hospital and in the community, to enable a smooth transition and consistency of care through the discharge process. Patients may require a discharge home visit and/or follow-up visits in the community to assess them in their own environment. The main focus is always to assess functional ability in a familiar environment where possible, to support people to regain their optimal level of functioning, and to achieve what matters to them.
Main duties of the job
You will be expected to treat and support patients who have been identified for the Early Supported Discharge (ESD) Team either as an inpatient or in the community. You will be expected to work autonomously, setting treatment goals, monitoring patient progress, encouraging independence with activities of daily living and adapting treatment as necessary. You will also be required to support the daily running of the ESD team and supervise junior team members while driving motivation and a positive ethos throughout the team.

The ESD team is a 7-day service from 08:00-18:00; therefore, flexibility in working pattern is required. A standard working week is 08:00-16:00. Having a full driving license and daily access to a car is essential.

Person Specification
Experience
* Broad clinical experience as a Band 5 therapist.
* HCPC registration.
* Able to demonstrate evidence of ability to work at a Band 6 level whether through current Band 6 role or through development and training within a Band 5 role.
* BSc honours degree or diploma in Physiotherapy or Occupational Therapy.
* Evidence of successful working within a multi-disciplinary team.
* Experience of working within a community setting.
* Evidence of advanced specialist knowledge in management of complex patients with Long Term Conditions which require advanced clinical reasoning.
* Experience of managing risk around discharge and an understanding of the importance of embracing risk to enable patient choice when planning hospital discharge.
* Post-graduate training/qualification/education in relevant subject area. E.g., masters modules, leadership courses.
