Monday - Friday - 37.5 hours per week.
Salary
This position is full time,youll receive an annual salary up to £50,000 per annum FTE - depending on experience.

Main duties of the job
As aRelease and Transfer Nurse, your responsibilities will vary. You will
* Use your expert clinical skills and demonstrate judgement in assessing and managing wide ranging and complex patient problems.
* You will manage the assessment of health needs, planning, delivery and evaluation of care, for the prison population, including clinical risk assessment.
* Provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment.
* Be expected to work collaboratively with the patient and any other agencies to support a safe transition back into the community/ other establishment.
* The Release and Transfer nurse will conduct a holistic patient assessment to include; Clinical, Psychological, Substance Abuse, Social, Educational, Training and Employment and Spiritual / Chaplaincy.
